Sensitive to the tremendous impact that giving up long-held homes may have on the lives of the men and women who move our assisted living facility, Daughters of Sarah Senior Community offers the services of a Social Worker to help residents with any emotional issues they may have during their transition to The Massry Residence.
We are committed to establishing and maintaining a strong relationship with the men and women who have chosen to make their homes here. We help them with all aspects of their transition.
We orient them to the building and activities schedule; make sure they meet fellow residents, and spend time with them as they adjust to their new lifestyle.
Residents can receive help with planning, linking resources in and out of the building, crisis intervention, and psychosocial support.
We’re also available by phone. In addition, we make ourselves available to their families through the very important transition period.
Case Managers maintain relationships with our residents as they adjust, and work closely with the activities director who gives numerous opportunities to facilitate social groups and events. They also talk to the residents in social settings, and make one-on-one visits to their apartments. They work closely with the Resident Care Director to determine when a resident is declining and needs to transition to a higher level of care.
One of the most important benefits for our residents is the ease with which a move into our adjacent Daughters of Sarah Nursing Center may be made if and when necessary. Of course, should a resident need to receive a higher level of care and desires to receive it elsewhere, we work with him or her and the family to help as much as we can.
The Social Services department works closely with the residents and their families to make each transition as smooth as possible.
